Financial assistance for patients at UF Health North, UF Health Jacksonville’s hospital campus in north Jacksonville, through the same UF Health Jacksonville assistance program. Patients who cannot pay may qualify for help based on income. Applications can be picked up in person at the UF Health North Admissions Department.

Get an application in person at UF Health North Admissions Department (15255 Max Leggett Parkway), UF Health Jacksonville Admissions (655 West 8th Street), or the Financial Eligibility Department (1833 Boulevard). Return the completed application to 1833 Boulevard in person, or by mail to ATTN: UF Health Jacksonville Financial Evaluation Department, 655 West 8th Street, Jacksonville, FL 32209. Cost
